Vote for the name of our new service

Take part in our quick online survey to help us choose a name for our new service, aimed at providing better care for people with long term conditions and complex needs in their own homes.

The Active Case Management (ACM) Service is being provided in partnership with Southport and Ormskirk Hospital and will begin operating in early in 2013 – initially with pilots at Chapel Lane, Roe Lane, Corner and Marshside surgeries.

Your GP practice

Your GP is available from 8am to 6.30pm weekdays with same day appointments available if necessary.

Ear, nose, throat and chest infections are common over winter. Your GP is there to help if these illnesses are persisting and you are getting concerned.

 You need a GP but your surgery is closed

 When the surgery is closed, you can still see a local GP with the GP out-of-hours service – just contact your practice number to access the service. Personal care when it just can’t wait

Your local pharmacy

Pharmacists offer free, friendly, confidential, expert advice and treatment for everyday health issues. They are open early till late, even on Christmas Day and Bank Holidays – you don’t need an appointment.

Sefton patient shows how the flu jab is quick and easy

Following a significant increase in flu cases across Merseyside, local PCT’s and GP’s are witnessing a welcomed surge in people attending specialist flu clinics. 

Christine Roberts, 62, visited her local clinic to receive the flu jab for the first time. She was advised to have the flu jab because she suffers from Chronic Obstructive Pulmonary Disease.  Often caused by smoking, the condition restricts the airflow to the lungs. 

Christine, who proudly quit smoking three and a half years ago, said: “I had no concerns at all about having the flu jab. Plenty of my older friends and family get it regularly and I’ve never heard any complaints. In fact it’s nice to know that I will be more protected this winter.” 

All individuals, like Christine, who have an underlying medical condition (whatever age) are eligible for a free flu jab. Residents who are pregnant or 65 or over are also entitled to receive the flu jab for free and are advised to do so before winter sets in. 

Pippa Rose, Practice nurse from NHS Sefton said: “It’s great to see that people understand the importance of getting the flu jab, and are taking action to prevent themselves from getting poorly and also to stop the spread of the virus in the community. 

“The special flu clinics we are running have been a great success so far as they are more accessible to our patients, who can be in and out in minutes. 

“Our patients come for their flu jab year on year, and all boast about how they enjoy a flu free winter and a healthy Christmas as a result.” 

If you’re eligible for the free flu vaccination, simply contact your local GP to find out about the regular flu clinics. Once you have been vaccinated you will be protected for the rest of the winter.

 

Award for Southport doctor

Dr Graeme Allan, from Southport and Formby Clinical Commissioning Group (CCG), has won a national award for his work to develop the new Macmillan Cancer Information Centre in Southport.

Graeme, from St Marks Medical Centre, was nominated in the Service Improvement Excellence Award category – for Macmillan professionals who have achieved definable improvements to the services offered to people affected by cancer.

The awards launched in 2011 and were developed to celebrate the outstanding work carried out by Macmillan professionals. There are over 5,000 Macmillan health and social care professionals across the UK, many of whom are specialists in their area of cancer care and leaders in their field.

As a GP and Macmillan GP, Dr Graeme Allan has led on the implementation of the Macmillan Cancer Information Centre in Southport. The centre is unique in having a community base within an independent living centre.

Dr Allan said: “I am delighted to receive this award on behalf of the cancer team of Southport and Formby Clinical Commissioning Group (CCG). It means a great deal to us as an emergent CCG to achieve this early success. The Southport Macmillan Information Centre has provided a much needed resource for patients and carers and it is testament to Macmillan that they were prepared to work in partnership with a brand new organisation and provide the assistance we needed to achieve our goal. I would like to pay tribute to everyone who has been involved, and through the centre, we continue to explore new areas of support to cancer patients and carers such as new in house courses and a walking group.”

Sonia Holdsworth, local Macmillan Development Manager at Macmillan Cancer Support, said: “This is only the second year we have run the Macmillan Excellence Awards, which offer a great opportunity to celebrate the exceptional contribution to cancer services made by Macmillan professionals. Each category has been carefully selected as being important in improving the lives of people living with cancer, and it’s fantastic to see the performance of a local professional being recognised on a wider scale.

“Dr Allan is part of a team of local Macmillan experts supporting people affected by cancer every step of the way. The charity is only able to fund roles such as this with the support of everyone who gives their time or helps to raise funds.  We can all make a difference by working together.

Talking Health and Wellbeing in Sefton

Everyone with an interest in health, social care and wellbeing in Sefton is invited to one of three events in January, where they can hear about plans for the future and give their views.

‘Talking Health and Wellbeing’ follows a number of public events held over the summer by Sefton Council, South Sefton Clinical Commissioning Group (SSCCG) and Southport and Formby Clinical Commissioning Group (SFCCG), where people were asked what they thought the priorities of the three organisations should be.

People’s views gained at these events have helped to shape joint proposals about which health, social care and wellbeing issues Sefton Council and the two CCGs should concentrate on.

The Talking Health and Wellbeing events are a chance to share these proposals with Sefton residents, gain their views and set out what the next steps will be.

Cllr Ian Moncur, Chair of Sefton’s Health and Wellbeing Board, said: “By setting joint priorities and doing more together, the Council and Clinical Commissioning Groups have a real opportunity to make a difference to the health and wellbeing of local people.

“We cannot do this without the views of Sefton residents and our Talking Health and Wellbeing events are one way they can get involved and tell us what they think about our plans for the future.”

Around 200 people attended the first two Talking Health and Wellbeing events in December and there are three more planned for January. People are welcome to register at any of the events but the one nearest to where they live will be most relevant to them:

  • 8th January  – 1.30pm (2pm start) Formby Gild Hall, Church Road, Formby
  • 14th January  – 1.30pm (2pm start) Crosby Lakeside Adventure Centre, Cambridge Road, Waterloo
  • 14th January  – 5.30pm (6pm start) Maghull Town Hall, Hall Lane, Maghull

Dr Niall Leonard, Chair of SFCCG, said: “People told us about some of the things they’d like to see in the future at events like the ‘Big Chat’. This is a chance for them to see how we are planning to respond to the things that matter the most to them, like offering more care closer to home where we can.”

“We are at an important stage in developing our plans to improve health and wellbeing locally, and these events are an excellent way for us to find out what Sefton residents think and help to further shape our proposals.”
